Loan & property
Your loan details, what you want to do with the property, and how it is used.

Borrower & co-borrower
Contact details for each person on the loan. Leave the co-borrower section blank if there is none.

Property status & other details
Sale listing, credit counseling, HOA fees, bankruptcy and military service.

Income
Monthly household income for all borrowers, before taxes. Totals update as you type.

Expenses & liens
Monthly household expenses and debt payments, plus any other liens on the property.

Assets
Household assets associated with the property and/or borrower(s), excluding retirement funds.

Income documentation
Tick what applies to you. The printed form — and your checklist — will show which documents to include.

Hardship affidavit
When your hardship began, how long you expect it to last and the primary reason — plus the documents you’ll provide.

Acknowledgement & signature
Read the certifications. You will sign and date the printed form by hand.
What you’ll need to hand
- ✔ A recent mortgage statement (loan number, servicer name, payment amount)
- ✔ Social Security numbers and dates of birth for each borrower
- ✔ Recent paystubs or other proof of income for everyone in the household
- ✔ Monthly bills: insurance, property taxes, car payments, credit-card minimums
- ✔ Bank, savings and investment balances
- ✔ Details and dates of your hardship, and any documents that support it
After you print
Sign and date the form in ink. Your servicer will usually also want an IRS tax-transcript authorization form, your income documents and your hardship documents — the checklist at the end of the wizard lists them.
Send the package the way your servicer asks (upload portal, fax or mail), keep copies, and write down every call in your servicer contact log. If you don’t hear back, follow up in writing.
